MERSEYSIDE
COMMUNITY CARE TEAM
The
Merseyside Community Care Team provides support to Social Services'
Service Users with enduring Mental Health diagnoses and/ or Learning
Disabilities in furnished self- contained flats in areas of choice.
The
properties are supplied by Adullam or 'peripatetic' support can be offered
to 'Service Users' in their own homes. We also have one shared house
with overnight staffing. Intensive support packages for individuals
with complex needs can be provided with staff sleeping on the premises.
Male
and female applicants eighteen years old and over need a Social Services
Social Worker to assess their needs for funding purposes as 'support
is funded' by allocations of 'spot funding' from 'Panel'. Social Services
and Health Authorities jointly fund some support packages.
'Care
planning' involving a named Project Worker, the Service User and his/
her Social Worker takes place within the framework of a 'Support Contract'.
Life and Social Skills mentoring takes place augmented by emotional
support and monitoring of a service users well being. Regular reviews
are held to monitor the effectiveness of the service delivery. The Care
Programme Approach is adhered to and the Team Manager attends CPA meetings.

AMINA

BELVEDERE
PROJECT - (Runcorn)

A
Housing, Support, Training and Education Project
Single
homeless men and women aged 16 - 25 years
We
work in close partnership with Halton Borough Council, Young Peoples
Team and many other agencies.
Referrals
come from a variety of agencies including the Local Authority of Halton,
Young Peoples Team, Youth Offending Team, Social Services, Drug Teams,
other RSL's etc.
Twenty-four
hour staff support, on site offices.
Self
catering accommodation
Nineteen
self contained flats and four bedsits. Each flat has its own kitchen,
bathroom, lounge. Communal facilities include lounge with colour TV
- which also acts as a smaller one to one/ small group work room of
a day time, training kitchen, laundry.
There
is also a well equipped Training Room and the Project has just secured
a further £250,000 of SRB 5 funding for the continuation of the training
and education over the next five years
Accommodation
is fully furnished and equipped, bedding and cleaning materials are
supplied.
Two
self-contained units of move-on accommodation are now to be applied
for using SRB Capital monies and ERDF funding ( Proposed to come on
stream Christmas 2001). We are about to recruit a Move On/ Resettlement
Worker funded from SRB 5

TO MERSEYSIDE PROBATION SERVICE.

to 49 Fully furnished and equipped self-contained and shared flats and
bed-sits spread across Liverpool, Sefton, Knowsley & St.Helens (bedding
supplied).
Facilities
include: fully fitted kitchen, laundry facilities, lounge & bed sitting
room, kitchen with colour TV, and most with full central heating. Monthly
cleaning packs supplied.
Tenure:
Assured Shorthold Tenancies and a Personal Charge is made to cover all
service bills.
Six
High Care places with dedicated Project Worker and Night Support Workers,
video surveillance recording for extra security.

Medium care dispersed properties and 21 low care with peripatetic Project
Worker from Monday-Friday (daytime).
All
Service Users are required to take part in a Personal Support Contract.

PROBATION PROJECT (CHESHIRE)

PROBATION SERVICE
Shared
supported housing in three, two bedroom houses, one property in Widnes
and two properties in Runcorn all with peripatetic support from staff
from Monday-Friday (daytime).
All
shared housing is fully furnished, with colour TV. Bedding and monthly
cleaning materials pack is provided. Lounge, kitchen, bathroom and laundry
facilities are shared. Tenants have their own room and a front door
and room key is supplied.
All
Service Users are required to take part in a Personal Support Contract.
Tenure: Assured Shorthold Tenancy and a Personal Charge is made to cover
all service bills.
